REGISTRATION FOR FALL 2008
Reminder: This is the Sparta
Soccer Club's form to gather the information required by MCYSA. The information that you
submit here goes to the SSC Director of Travel Teams for review
before it is forwarded to the league. Coaches ARE NOT to
submit directly to MCYSA Questions about team

forwarded to the league by 7/1/08

Submissions with incomplete data will not be accepted and team will not be
registered.
Teams not registered as of July 1, 2008 will not be eligible to participate in the
FALL 08 season.

INFORMATION ~
We need at least TWO
different contacts for your team
- Primary Contact and Alternate Contact.
Both contacts
must be different people, have separate phone numbers and email
addresses OF THEIR OWN,
and may not be members of the same household.
Otherwise it
defeats the purpose of having a back up contact! The Primary and Alternate
contacts are published on the website along with their email, phone, and fax
(if available). You can designate the Coach as either the Primary or
Alternate contact, however some Coaches or Coach/Trainers prefer to appoint
the Team Manager, Asst. Coach, Team Parent, etc. as team contacts.

If information for

the registration can not be accepted

IMPORTANT! COMMENTS
FOR FLIGHTING CONSIDERATION ~ Include information to substantiate why you
are requesting the above flight level and/ or any information that will
help assure the proper flighting for this team
Please
be as DETAILED as possible with any pertinent comments that should be considered when flighting your team. The flighting committee heavily weights your comments. The number of
flights in an age bracket varies from season to season and can not be determined until all
registrations have been received.
** By requesting a specific flight # at this time you
may cause your team to be mis-flighted.
** Therefore, please
do NOT request specific flight numbers base your comments on the Flight Levels
listed above.
* IMPORTANT *
League Guidelines prohibit scheduling of games before 1 pm on
Sundays without prior consent of the visiting team. Some teams do
not have a problem with games scheduled early than 1 pm.
Please indicate the EARLIEST time a game for this team can be
scheduled.
(It is expected
most games will still be scheduled after 1 pm.) |
