TOWN LEAGUE REFEREES PAYMENT REQUEST FORM  SPRING 2008

Instructions:

 Please fill out the form below for each game that you referee.  Use only one line for each game.  The treasurer will issue checks only once this season.  It is very important that you fill out each line completely and send the completed form to me by the date listed below.    I will verify the information you provide so a check can be issued to you.  If there are any problems or questions let me know.        DEADLINE FOR PAYMENT REQUEST: JUNE 22 (Very important! -you must mail (in Sparta) or give to Mr. O'Leary on or before 6/21/08 or you WILL not get paid   TREASURER CLOSES THE BOOKS!!!  

Please indicate whether there were two refs at your game or only 1 (note - you will be paid 1.5 times the normal rate for games refereed alone)

Rates: Div. 3, 4 -  $15.00   (Alone $22.50)           Div 5, 5G - $13.00 (Alone $19.50)       Div. 6A, 6B, 6G - $12.00  (Alone 18.00)

If you cannot referee a scheduled game, you must get your own substitute for that game.  Look on the referee list and ask someone to take your game.
